Social Worker

Job description

We are delighted to be working in partnership with a local charitable organisation to recruit for an experienced Social Worker to join their palliative care team.

This role is to provide a specialist palliative social work service to patients and their families, encompassing both pre and post bereavement care. Monday to Friday 37.5 hours per week is on offer for the suitable candidate.

Responsibilities
To deliver specialist palliative care social work interventions for those with identified complex holistic needs.

To work as part of the multi-disciplinary team and to represent social work in relevant meetings.

To advise and participate in the management of complex issues being experienced by patients and families, regarding psychological, emotional and social issues.

To provide practical advice and signposting to patients, carers and families on supportive resources available in the community

To maintain written records and statistics in line with Data Protection Policies and Procedures and NISCC Guidelines.

To actively contribute to the social work bereavement programme, offering support to adults, children and young people, both individually and in groups

Essential Criteria
Recognised Social Work qualification.

Registered with the NISCC

A minimum of 3 years recent post-qualifying experience in a health related field

2 years recent experience of working with people who have life-limited conditions and/or palliative care needs
Ability to work independently and as part of a team.
