An exciting opportunity has arisen for a Pharmacist to provide full time temporary cover in a Hospital environment, the role is for 37.5hrs per week. This is a great chance to develop skills & experience within a large healthcare setting and to work as part of a larger team delivering both long term and acute medication services.
KEY DUTIES / RESPONSIBILITIES
Dispensing and Distribution
- Participate in the work of the dispensary, including clinical and accuracy checking of prescriptions and dispensed medicines, both within the dispensary and at ward level.
- Review and assess inpatient and outpatient prescriptions for legality, accuracy, safety and appropriateness of drug therapy.
- Ensure all work completed complies with Medicines Ethics and Practice, local policies and procedures, risk management and health and safety policies and protocols.
- Communicate recommendations on drug therapy to doctors, nurses and other relevant healthcare professionals.
- Liaise with relevant clinical pharmacists on patient care issues.
- Dispense and supply medicines.
- Advise patients on concordance with their medicines.
- Receive, store and supply controlled drugs in accordance with the Misuse of Drugs Act and Trust policy.
- Provide a professional and courteous service to patients, staff and other healthcare staff attending the dispensary.
- Participate in the enquiry answering service of the Medicines Information service under the direction of a more senior pharmacist, acting in accordance with UKMI Standards and local standards of practice.
- Undertake training involving relevant computer assisted learning packages and accredited Medicines Information workbooks under the supervision of a trained Medicines Information Pharmacist.
Aseptics and Quality Control
- Work in the Aseptic unit (including radio-pharmacy if required), collaborating with other pharmacists and senior technicians in the prioritisation of workload and the provision of a safe, efficient and timely service.
- Undertake the in house training programme on the technical aspects of aseptic manipulation and the clinical aspects of aseptic products e.g. parenteral nutrition, intravenous additives and chemotherapy.
- Develop knowledge and understanding of current legislation and guidance, standards of practice, COSHH, GMP, risk management and Health and Safety policies governing aseptic preparation.
- Ensure all work complies with standards of practice, risk management and health and safety policies and protocols including the handling of cytotoxic drugs.
- Review and assess chemotherapy and parenteral nutrition patients’ prescription charts for legality, accuracy, safety and appropriateness of drug therapy.
- Check the accuracy of assembled aseptic medicines for individual patients.
- Work in the quality control section of the pharmacy department.
Clinical Pharmacy and/or Integrated medicines management
- To assist in the provision of a clinical pharmacy and/or integrated medicines management service to allocated wards.
- To obtain comprehensive admission medication histories and rationalise patients’ medication on admission.
- To ensure the appropriate management of patients’ own drugs.
- To provide a comprehensive medication monitoring service for inpatients and optimise medication use in allocated wards.
- To carry out discharge counselling as appropriate.
- To prepare documentation relating to medication changes carried out during the hospital stay.
- To prepare the discharge prescription and ensure that timely discharge information is available to primary care healthcare professionals.
- To further develop and roll out the generic switching and therapeutic tendering changes at ward level if required.
- To participate in the supply and dispensing of pharmaceutical products and the provision of patient services.
- To provide medicines information to patients, carers and other healthcare professionals.
- Deputise for pharmacists managing other areas of pharmaceutical activity and manage professional, technical and ancillary staff as required.
- Act as the Responsible Pharmacist when required.
- Help provide cover for pharmacists working in other hospital pharmacy departments within the Trust.
- To comply with all relevant CPD requirements.
- To provide a service in accordance with published standards and local policies and ensure safe systems of working and compliance with written procedures.
- To ensure the completion and maintenance of appropriate documentation and records.
- To ensure that the output and quality of the service is maintained.
- To participate in shift working, bank holidays, weekend working and extended working hours in accordance with relevant rotas.
The post holder will be required to:
- Co-operate fully with the implementation of the Trust's Health and Safety arrangements, reporting any accidents/incidents/equipment defects to his/her manager, and maintaining a clean, uncluttered and safe environment for patients/clients, members of the public and staff.
- Adhere at all times to all Trust policies/codes of conduct.
- Comply fully with the Trust’s policy and procedures regarding records management, as well as the Data Protection Act, accepting legal responsibility for all manual or electronic records held, created or used as part of his/her duties, and ensuring that confidentiality is maintained at all times.
- Represent the Trust’s commitment to providing the highest possible standard of service to patients/clients and members of the public, by treating all those with whom he/she comes into contact in the course of work, in a pleasant, courteous and respectful manner.
QUALIFICATIONS / EXPERIENCE
- M.Pharm in Pharmacy or equivalent.
- Membership of the Pharmaceutical Society of Northern Ireland or eligible for membership.
- Hold a full current driving license valid for use in the UK and have access to a car on appointment.
 This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving but who is able to organise suitable alternative arrangements in order to meet the requirements of the post in full.
