Clinical Pharmacist Specialist
Hospital Setting
$59.06 to $62.55 per hour, alongside leading health benefits, professional development allowance and RRSP matching.
MPA Recruitment has been retained by a leading hospital and care complex to recruit a Clinical Pharmacist Specialist to join their team. Our client is a world-class acute care facility with 375 beds, situated on a landscaped site of nearly 70 acres the hospital is beautifully architecturally designed. Reporting into the Pharmacy Manager, Clinical Pharmacist Specialist (CPS) will be committed to quality improvement processes, mentorship, education and perform critical decision-making regarding medication management. In addition, the pharmacist is actively involved on interprofessional teams to provide exceptional care to patients.
Responsibilities.
Provide patient care or services in a manner that reflects the philosophy of patient and family centred care.
Be accountable for ensuring the safest patient care and services and for recognizing and reporting safety incidents or potentially unsafe conditions.
Lead and be responsible for minimum of one quality improvement process per year including projects, committee participation, and policy development. This process improvement will be approved by Pharmacy Leadership
Assess and manage medication shortages in collaboration with pharmacy leadership.
Provide pharmacy related education or presentations to stakeholders at least yearly.
Pharmacotherapeutic services including but not limited to complete medication reconciliation; develop and initiate a pharmaceutical care plan, identify and resolve drug therapy problems, participate in interprofessional patient care rounds to improve medication management, provide education to patients about their disease(s) and medication(s), review and dispense medication orders, resource to healthcare professionals.
Participates in the orientation of new staff (pharmacy, physicians, and nursing) and training of student learners.
Maintain professional competence by keeping informed of changes in medical practice, new drugs, and their role in current therapy.
Maintain workload measurement statistics.
Participate in the implementation of quality and risk management programs (Adverse Drug Reaction reporting and Medication Near Miss and Incident Reporting).
On-call duties as assigned.
Essential Criteria
Doctor of Pharmacy or Bachelor of Science in Pharmacy with Accredited Canadian Pharmacy Residency (ACPR)
ORBachelor of Science in Pharmacy and 2 years of relevant acute care hospital pharmacist experience
Must be Registered Pharmacist Part A with Ontario College of Pharmacists

Previous acute care hospital pharmacist experience
